We are seeking a dedicated Mental Health Social Worker to join our team in Elberton, Georgia. As a key member of our clinical team, you will play a vital role in promoting proactive measures to enhance patient care standards and improve CMS ratings.

Responsibilities:

Provide mental health services to patients, including assessments, counseling, and case management Collaborate with interdisciplinary teams to develop and implement personalized care plans Identify and address social determinants of health that impact patient outcomes Utilize evidence-based practices to deliver high-quality patient care Maintain accurate and timely documentation of patient interactions and services rendered Participate in quality improvement initiatives to enhance patient outcomes and reduce hospitalizations

Requirements:

Master's degree in Social Work (MSW) or related field Licensure as a Licensed Clinical Social Worker (LCSW) or eligibility for licensure trong communication and interpersonal skills Ability to work effectively in a fast-paced environment with multiple priorities Familiarity with electronic health records and practice management systems
