  • Year round, affordable on-site childcare at KCH Kid's Club At Kaniksu Community Health, the Behavioral Health Case Manager is not just responsible for providing excellent customer service.
They are committed to working closely with Behavioral Health care providers to help identify what type(s) of additional health/social services an individual is in need of and connects the patient to these supports. YOU ARE an essential team member charged with assisting patients in managing their care at Kaniksu Community Health. Under the direction of the KCH Director of Behavioral Health and Behavioral Health Providers and as a member of the Kaniksu Community Health Patient Centered Medical Home ( PCMH ) Team, a typical day of the Behavioral Health Case Manager might include;
  • Provides appropriate triage support for individuals identified by medical or mental health providers as being in need of supportive health/social services.
  • Works as a treatment team member.
  • Provides crisis response and support when needed.
