at Chinese American Service League, Inc. in Chicago, Illinois, United States Job Description Reporting to Alzheimer's Program Coordinator, work as a member of the Alzheimer's Program staff on screening the cognitive health of clients and work closely with all program departments to assess entire household behavioral health need while providing clinical intervention, including counseling, behavioral management, case management, and care coordination.

Duties:

Provide clinical assessment and interventions under medical licensed clinical supervision; screen the cognitive health needs of clients using validated/standardized screening and assessment tools; provide case management and care coordination to clients with mild cognitive impairment, dementia, or other cognitive impairment symptoms and their families to make sure they are connected with the right resources and specialists for diagnostic evaluation; participate in educating community members on dementia care, behavioral health, and trauma-related knowledge; maintain documentation of all the clinical work, workforce training, and community workshops and provide reporting to supervisors and external parties regularly; participate in and contribute to department team meetings, continuing training sessions, external partnership meetings, teleconferences, roundtables, and other important meetings.
