General Functions:
Provide housing supports and mental health services to individuals served in the agency's Permanent Supportive Housing program. Population of focus is homeless adults with a serious mental illness and/or co-occurring substance use disorder.
Requested Specific Functions:
Provide case management and housing support services in individuals' homes and in the community in accordance with an evidence-based Housing First model. Make regular home visits to individuals (once per month at minimum) to provide support and assistance needed to maintain housing Assist with relocation, as approved, including packing belongings, coordinating moves and ensuring residents' homes are in good order at move-out. Participate in evidence-based practices training (i.e. Motivational Interviewing, Harm Reduction Techniques, etc.) and utilize the learning to engage individuals and keep them safely housed. Deliver facility and community-based voluntary mental health services in accordance with DMH Rule 132 and HFS Rule 140 as indicated on the individuals' treatment and recovery plans, including but not limited to individual and group community support services. Record the process and results of services within established timeframes and in sufficient detail to meet DMH Rule 132, HFS Rule 140, and/or other insurance billing requirements. Meet or exceed assigned MRO weekly billing quota as determined by the Director. Assist individuals with budgeting, home maintenance, increasing socialization, building natural supports, managing symptoms of mental illness, managing stress, managing medications, accessing community resources, managing and improving health and/or improving overall wellness and quality of life, as indicated in individuals' personalized treatment and recovery plans. Follow department protocols regarding crisis intervention, home well-being checks, documentation, and communication regarding status of individuals on caseload. Participate in monthly clinical supervision meetings; seek ongoing clinical supervision from supervisors; and participate in continuing education to maintain applicable licenses/certifications. Participate in all agency and department trainings, monthly department meetings, and any additionally recommended trainings or meetings as required by supervisor(s). Provide telephonic-only crisis intervention services in accordance with the staff rotation on the weekly crisis line schedule. Maximum of two weekly rotations per year. Crisis phone provided. Complete any other related duties as assigned by the Community Housing Case Manager, Lead Case Manager, and Coordinator of Community Housing.
Qualifications:
Consistent with Medicaid Rule 140 requirements for either Mental Health Professional (MHP) or Rehabilitative Services Associate (RSA). Candidates must be at least 21 years of age and be a high school graduate or have a GED certificate. An MHP must possess a bachelor's degree in counseling, psychology, social work, or a related field, or possess a practical nursing license, or be a Certified Recovery Support Specialist (CRSS), or be a licensed occupational therapy assistant with at least one year of mental health experience or have a minimum of five years supervised experience in mental health/human services. If an individual possesses a bachelor degree in a field other than a human services related field, the individual must have two years of documented clinical experience in a mental health setting under the supervision of a QMHP. An RSA must have demonstrated skills in the field of services to adults or children, demonstrate the ability to work within an agency's structure and accept supervision, demonstrate the ability to participate as a member of an inter-disciplinary team and demonstrate the ability to work constructively with adults receiving services, other service providers, and community members. Proficiency in the use of computers and typing; and basic working knowledge of MS Word, MS Excel, and MS Outlook applications required. Driving record and auto insurance coverage in accordance with agency policy. A post-offer physical and criminal background check required. Reliable transportation required. EOE.
