_PARAGRAPH JOB_DESCRIPTION.SHARE.HTML Jeffersonville, Indiana Licensed Clinical Social Worker 159855 Regular Full-Time Days Norton Clark Hospital mail_outline Get future jobs matching this search Login or Register Job Description Responsibilities The Social Worker is a member of the multidisciplinary care team to meet the following patient/family significant other needs: response to psychosocial and psychodynamic needs; coordination of community resources to ensure the transition to the least restrictive care environment, coordination/facilitation of discharge planning, seamless continuity of care process, therapeutic intervention and financial counseling. The Social Worker interfaces directly with patient/family/significant other and all other interdisciplinary members of the care team. The Social Worker provides short-term counseling and crisis intervention to patients and families identified as having complex social services needs, including but not limited to; emotional/psychiatric issues, family violence or abuse, legal issues or financial problems.

Qualifications Required:

One year social work or One year social work internship. Must have hospital and/or payer credentialing completed before functioning in this role.

Master Degree Licensed Clinical Social Worker Desired:
