Targeted Case Manager (TCM) Position Summary Titans and Associates LLC is seeking a compassionate, organized, and self-motivated Targeted Case Manager (TCM) to provide community-based case management services to children, adolescents, and families receiving Kentucky Medicaid services. The Targeted Case Manager will coordinate services, advocate for clients, connect families with community resources, and assist individuals in accessing medically necessary behavioral health, medical, educational, and social services. This position is ideal for professionals who value flexibility, enjoy working independently, and are passionate about improving outcomes for children and families through person-centered, strengths-based care. Essential Duties and Responsibilities The Targeted Case Manager will: Conduct comprehensive assessments to identify client strengths, needs, and barriers to treatment. Develop, implement, and monitor individualized Plans of Care in accordance with Kentucky Medicaid requirements. Coordinate behavioral health, medical, educational, social, and community-based services. Maintain regular contact with clients, families, providers, schools, and community partners. Monitor client progress and update Plans of Care as clinically indicated. Assist clients and families in accessing housing, transportation, food assistance, educational supports, healthcare services, financial resources, and other community programs. Advocate on behalf of clients to reduce barriers and improve access to medically necessary services. Collaborate with multidisciplinary treatment teams to ensure coordinated care. Complete all documentation accurately, timely, and in compliance with Kentucky Medicaid regulations. Maintain confidentiality in accordance with HIPAA and agency policies. Participate in required supervision, quality assurance activities, and agency meetings. Minimum Qualifications Applicants must meet Kentucky Medicaid qualifications for Targeted Case Management.
Preferred qualifications include:
Bachelor's degree in Social Work, Psychology, Counseling, Human Services, Sociology, Criminal Justice, or another related behavioral science field. Minimum of one (1) year of experience working with children, adolescents, or families. Experience providing Targeted Case Management is preferred. Knowledge of Kentucky Medicaid regulations and documentation requirements is preferred. Strong written communication and documentation skills. Excellent organizational, problem-solving, and time management skills. Ability to work independently while maintaining accountability. Reliable transportation, valid driver's license, and current automobile insurance. Required Training Applicants must have successfully completed Kentucky Targeted Case Management (TCM) training or be willing to complete the required training prior to assuming an active caseload. Titans and Associates LLC will cover the cost of the required TCM training for qualified applicants. Performance Expectations Successful contractors are expected to: Maintain an active caseload of up to 25 families in accordance with Kentucky regulations. Complete all required monthly case management activities and service coordination. Submit documentation within 24 hours of service delivery whenever possible. Maintain documentation that meets agency quality assurance standards and Kentucky Medicaid requirements. Respond to referrals, client needs, and provider communications in a timely and professional manner. Demonstrate professionalism, ethical conduct, and person-centered service delivery. Maintain effective communication with supervisors and agency staff. Attend required supervision, training, and quality improvement meetings. Build collaborative relationships with schools, healthcare providers, community agencies, and families.
