Job Description

Mountain Comprehensive Care Center is hiring for a Targeted Case Manager based in Owensboro and will serve the Daviess and surrounding counties .
PURPOSE/RESPONSIBILITIES
To assist individuals with severe mental illness (SMI), severe emotional disorder (SED), substance use disorder (SUD), or chronic and complex physical health with behavioral (BHPH) in negotiating for needed services and resources in the community and mental health settings.
MINIMUM QUALIFICATIONS
BA or BS in any field of Behavioral Science or Human Services from an accredited institution. Minimum of one year experience in case management or working with the severely mentally ill and one year experience working with adolescents. A Master's degree in a human services field may be substituted for the one year of experience. Must complete the case management certification program offered and approved by the DMHMR within 6 months of employment. Must have a valid Kentucky motor vehicle operator's license.
SPECIAL KNOWLEDGE, SKILLS AND ABILITIES
Knowledge of and personal experience with public mental health services. Strong reading comprehension, verbal and written communication skills. Demonstrates the ability to communicate with others in a professional manner.
