The Case Manager II provides comprehensive, trauma-informed case management services to individuals experiencing moderate levels of crisis, homelessness, or instability. This mid-level position supports clients with more complex needs through coordinated care, supportive counseling, and connection to critical community resources. The Case Manager II plays a key role in helping individuals build stability, resilience, and long-term self-sufficiency while upholding the mission and values of Catholic Charities of Acadiana. This role requires strong professional boundaries, compassion, and the ability to work effectively across systems and disciplines. Key Responsibilities Client SupportCase Management Provide trauma-informed case management to Tier 2 clients with moderate service needs Conduct strengths-based assessments and support individualized goal planning Deliver supportive, non-clinical counseling including motivational interviewing, emotional support, and de-escalation Connect clients to housing, behavioral health, medical care, employment services, and benefits Conduct field-based visits in shelters, community settings, or client residences as needed Care PlanningCoordination Develop individualized service plans with clear, measurable goals Coordinate care with internal departments and external service providers Support clients in navigating complex systems including healthcare, behavioral health, and housing resources DocumentationCompliance Maintain accurate, timely documentation in accordance with agency standards Track client progress, outcomes, and service metrics for reporting and quality improvement Crisis SupportProblem Solving Provide support during periods of increased client stress or instability using trauma-informed approaches Identify cases requiring escalation to higher-level case management or clinical services Participate in safety planning and coordinated response efforts as needed ProfessionalismCollaboration Maintain confidentiality, ethical standards, and professional boundaries Participate in team meetings, case consultations, and ongoing training Represent and uphold the mission and values of Catholic Charities of Acadiana Job Qualifications Bachelor's degree in Social Work, Psychology, Human Services, or related field preferred (Master's preferred) Minimum 1 year of case management or social services experience required Experience in homelessness, behavioral health, or crisis environments strongly preferred Strong understanding of trauma-informed care and harm reduction principles preferred Excellent communication, organizational, and documentation skills Ability to build rapport and maintain professional boundaries with diverse populations Proficiency with electronic documentation systems and Microsoft Office Suite Knowledge of local community resources, public assistance programs, and housing systems
Job Specifications Heavy:
Exerting 50 to 100 pounds of force occasionally, or 25 to 50 pounds of force frequently, or 10 to 20 pounds of force constantly to move objects. Physical demand requirements are in excess of those for Medium Work.
