Qualifications Required Qualifications:
Minimum of 2 years' experience in case management, direct support, or a related human services role. Knowledge of person-centered planning, developmental disabilities, and community resources. Strong organizational, documentation, and communication skills. Ability to work independently while maintaining collaborative relationships with teams and caregivers, and community partners. CPR/First Aid Certified Trainer (or ability to obtain certification within designated timeframe). Valid driver's license, reliable transportation, and willingness to travel extensively.Please note:
