- 4:30 pm EST Job Summary Responsible for receiving, coordinating, and providing services to all Targeted Case Management referrals.
Work Experience Education & Training:
Currently licensed asLCSW/LCPC
in the state of Maryland.Work Orientation & Experience:
Experience in Psychiatric Case Management, resources in Harford County and Critical Diagnosing preferred. Basic computer skills required.Skills & Abilities:
Demonstrate skill in a) psychiatric clinical case management and assessment of psychiatric patients; b) performing complete biopsychosocial assessments; c) effective critical thinking; d) written and oral communications; and e) age appropriate interpersonal interactions. Demonstrate knowledge of f) Medicaid regulations; g) acute care criteria and h) case management principles. Demonstrate ability to i) communicate and collaborate effectively with both internal and external customers; j) assess, adapt, and calmly respond to changing and/or crisis environment; k) make independent decisions consistent with current policies, procedures, and ethical standards; m) prioritize work assignments and manage time effectively to complete duties; and l) be available to adapt hours to meet the needs of all customers. Requires knowledge and use of the appropriate computer systems to communicate effectively with both internal and external customers.
