Job Description
Position Summary The Residential Case Manager serves as the primary coordinator of psychosocial and community support services for individuals receiving residential behavioral health and substance use treatment. This position assists clients in achieving stability, independence, and successful community reintegration by coordinating treatment planning, discharge planning, housing, employment, benefits, and linkage to community resources. The Residential Case Manager works collaboratively with therapists, medical providers, peer recovery specialists, nursing staff, and external agencies to ensure clients receive comprehensive, person-centered care throughout their treatment journey. Essential Responsibilities Client Advocacy & Community Support
- Advocate on behalf of clients to ensure timely access to behavioral health, medical, housing, vocational, and social support services.
- Assist clients with applying for and maintaining public benefits and entitlements, including Medicaid, Medicare, SNAP, SSI/SSDI, Temporary Cash Assistance (TCA), and other eligible federal, state, and local assistance programs.
- Coordinate and facilitate referrals to community resources, including housing providers, employment services, educational programs, legal assistance, financial counseling, and recovery support organizations.
- Arrange and coordinate transportation for medical, behavioral health, court, employment, and community appointments.
- When appropriate and in accordance with agency policy, transport clients to appointments, community meetings, and other essential services.
- Accompany clients to appointments when advocacy or care coordination is necessary to support successful treatment outcomes.
- Advocate for clients with healthcare providers, government agencies, insurance companies, employers, landlords, and community organizations to reduce barriers to care and promote client independence.
- Assist clients in obtaining identification documents, birth certificates, Social Security cards, insurance information, and other documentation necessary to access services.
- Educate clients on available community resources and empower them to independently navigate support systems.
- Develop and maintain collaborative relationships with community agencies and referral partners to improve continuity of care. Case Management
- Maintain an assigned caseload of residential clients.
- Complete comprehensive psychosocial and needs assessments.
- Develop individualized service plans in collaboration with the interdisciplinary treatment team.
- Monitor client progress toward treatment goals.
- Update documentation according to COMAR, Medicaid, and organizational requirements.
- Monitor client progress and proactively address barriers to treatment, housing, employment, transportation, and community integration.
- Maintain regular contact with family members, guardians, referral sources, and community providers, as appropriate and consistent with confidentiality requirements.
- Conduct community-based case management services when clinically appropriate. Care Coordination
- Coordinate treatment with therapists, psychiatrists, nurses, probation officers, DSS, parole officers, and community providers.
- Facilitate referrals to outpatient treatment, PRP, IOP, housing programs, vocational services, and recovery support.
- Coordinate medical appointments and specialty referrals.
- Assist clients with transportation planning when appropriate. Discharge Planning
- Begin discharge planning upon admission.
- Coordinate safe discharge placements.
- Connect clients with community-based providers.
- Schedule follow-up appointments prior to discharge.
- Ensure continuity of care documentation is completed. Documentation
- Maintain accurate electronic health records.
- Complete progress notes within required timeframes.
- Document case management interventions.
- Participate in utilization review and quality improvement initiatives. Team Collaboration
- Attend multidisciplinary treatment team meetings.
- Participate in case conferences.
- Communicate significant client changes to the clinical team.
- Assist during crisis situations when needed. Minimum Qualifications Required Education
- Associate degree in one of the following: Human Services Social Work Psychology Counseling Sociology Behavioral Sciences Criminal Justice Public Health Or a related field Preferred
- Bachelor's degree in Social Work, Psychology, Counseling, Human Services, Sociology, or a related behavioral health field. Experience Required
- Minimum 2 years of behavioral health, substance use, residential treatment, or community mental health experience.
- Experience working with adults experiencing co-occurring disorders. Preferred
- Residential treatment experience.
- PRP experience.
- Care coordination experience.
- Knowledge of Maryland behavioral health systems. Preferred Certifications One or more of the following:
- ADT
- CSC-AD
- CAC-AD
- CPRS Knowledge & Skills
- Motivational Interviewing
- Trauma-Informed Care
- Recovery-Oriented Systems of Care
