The CSW completes psychosocial assessments, provides education, advocacy, counseling, and support, and helps clients connect to behavioral health services. This role maintains a caseload and works closely with Community Health Nurses (CHN) and Community Health Workers (CHW) to assess psychosocial needs and determine appropriate emergent behavioral health services. The CSW also supervises the CHW embedded in the Emergency Department.

JOB SUMMARY
The Community Social Worker (CSW) is a member of the Hospital's Community Care Team (CCT), a program that supports Howard County residents with complex chronic illness. The CCT helps clients connect with resources to better manage their conditions, improve health outcomes, and reduce preventable hospital visits.The CSW completes psychosocial assessments, provides education, advocacy, counseling, and support, and helps clients connect to behavioral health services. This role maintains a caseload and works closely with Community Health Nurses (CHN) and Community Health Workers (CHW) to assess psychosocial needs and determine appropriate emergent behavioral health services.
