Hours/week Job Responsibilities:

Supervise and coordinate the delivery of mental health case management services at SMCDRC. Oversee mental health case management activities and ensure services are appropriately coordinated for the incarcerated/detainee population. Coordinate with healthcare professionals, detention center staff, and external providers regarding mental health needs and services. Monitor continuity of mental health care, including assessment, referrals, treatment coordination, and follow-up. Participate in the Classification Committee and Interdisciplinary Team as required. Support coordination of mental health services during admission, transfer, discharge, and transition to community-based care. Ensure appropriate mental health documentation, case records, referrals, and required reporting are maintained. Coordinate with the After-Care Planning Case Manager and other healthcare personnel to support continuity of care. Support compliance with applicable Maryland requirements, NCCHC standards, and SMCDRC procedures. Communicate identified mental health needs, service concerns, and required follow-up to the appropriate healthcare and detention center personnel.
