$1000 Bonus upon hire and $1000 Bonus at 1-year.
ESSENTIAL FUNCTIONS
Adult Community Case Managers have monthly contact with the assigned case load. Adult Community Case Managers will develop and monitor the Person-Centered Plan process. This includes but is not limited to scheduling, meeting facilitation, team coordination, and plan development. Adult Community Case Managers will complete progress notes/documentation. Adult Community Case Managers will meet all billable productivity standards. Adult Community Case Managers will ensure timely and accurate completion of all comprehensive assessments, quarterly reviews, and other related procedures. Adult Community Case Managers will provide linkage to services to meet identified barriers through referral, advocacy, and monitoring of services. Adult Community Case Managers will provide support to members to access needed resources. Adult Community Case Managers will provide advocacy for members, prompt self-advocacy, assist them with making choices, achieving identified goals, and promoting community inclusion. Adult Community Case Managers will maintain members records in the electronic medical record. Adult Community Case Managers is responsible for their own schedule.
JOB REQUIREMENTS
Bachelors Degree in Social Work or closely related field: Psychology, Mental Health and Human Services, Behavioral Health, Behavioral Sciences, Social Work, Human Development, Special Education, Counseling, Rehabilitation, Sociology, or Nursing 1-year paid or volunteer experience in the field of Social Work or closely related field listed above Valid driver's license with clean driving record and insured vehicle Reliable transportation Excellent verbal and written communication skills Ability to work independently under limited supervision Strong professional boundaries

$22.50 - $24.00 per hour
