Job Description

We are seeking a Case Manager/BHC to work in the RSU 71 Schools and with residents of Waldo County . This rewarding position functions as a part of a Behavioral Health Home team and includes helping clients/guardians assess and understand needs, coordinating referrals for support and treatment services, empowering clients to learn the skills to self-advocate and walking alongside the people we support to provide hope and celebrate successes. Working within a multi-disciplinary team that includes Peer Support staff and Nurse Care Managers, you will provide comprehensive case management services to improve the mental and physical health as well as the overall wellbeing of the clients you support. The yearly salary scale starts at $50,482.00 for entry level and is adjusted upwards based on each individual candidate's previous work experience. Our full-time roles offer great benefits such as a generous financial incentive program in addition to salary, a $2,500.00 sign-on bonus , flexible schedule that can include a 4 ten hour day weekly schedule, tuition reimbursement, health/life/vision/disability insurance plans, strong group and individual supervision, a warm team atmosphere and much more.
Requirements :
Bachelor's Degree Certified as Mental Health Rehabilitation Technician (MHRT) or eligible to obtain Driver's license in good standing with access to insured vehicle
Responsibilities :
Build therapeutic relationships with clients and caregivers through face-to-face interactions - including both in person and virtual meetings. Work with clients to understand needs and develop a plan of care tailored to their needs and preferences. Find and refer clients to resources to help meet their needs. Coordinate and facilitate team meetings with client care team. Communicate effectively with a wide range of clients who have individualized needs, with collateral contacts and with the BHH Leadership team about high-risk scenarios. Provide unconditional support and hope for clients working through challenging situations and help clients recognize and honor their successes.
Pay:
$50,482.00 - $64,272.00 per year
