Job Description

$1,000 signing bonus! Provides intensive, short-term supports to individuals with acute psychiatric symptoms in order to avoid more restrictive care settings and to maximize opportunities for recovery and desired quality of life. Functions as a part of an interdisciplinary team serving adults with serious mental illness. Participates in the Person Centered Planning team to assist in the coordination and provision of services to individuals within the Agency and the community. Responsible for strength-based recovery assessment, plan development, linking, referring, coordinating, monitoring medications and symptoms, advocacy and administrative duties in support of empowerment and self-determination. Assists in the development and implementation of crisis/safety/relapse plan. Qualifications Minimum of a bachelor's degree in social work, psychology, or related human services degree. Current Michigan social work licensure preferred. 3-5 years of experience providing service to persons who experience a serious mental illness and/or co-occurring disorders required. Must have a valid, unrestricted driver's license and availability of reliable transportation for outreach work.
