$4,000 RETENTION BONUS Segue, Inc. is a non-profit outpatient mental health facility that has been providing quality mental health services to the citizens of Jackson & Hillsdale counties in Michigan for over 30 years, offering intensive and essential services and programs. We are searching for a qualified ACT Team Lead for our Jackson, MI, office.
POSITION SUMMARY
The Mental Health Advocate provides person-centered, community-based behavioral health services to individuals receiving Assertive Community Treatment (ACT) services. This position promotes recovery, independence, and community integration through assessment, advocacy, care coordination, crisis intervention, skills development, referral and linkage, and ongoing support. Working as part of a multidisciplinary ACT team, the Mental Health Advocate partners with consumers, their natural supports, and community providers to develop and implement individualized treatment plans that promote improved health outcomes and quality of life. Success in this position requires excellent interpersonal communication, sound clinical judgment, effective time management, accurate documentation, collaboration within a multidisciplinary team, and a commitment to person-centered, recovery-oriented care.
QUALIFICATIONS
Required:
Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Criminal Justice, Nursing, Education, or related field Qualified Mental Health Professional (QMHP) or ability to meet QMHP qualifications under Michigan Medicaid requirements Valid Michigan driver's license with an acceptable driving record Current automobile insurance meeting Michigan requirements Ability to safely transport consumers in personal or agency vehicles Ability to maintain confidentiality Ability to document services accurately in an Electronic Medical Record Reside within a 45-minute radius of the local hospital to ensure readiness for possible on-call duties

Experience in community mental health ACT experience Crisis intervention experience Knowledge of person-centered planning Motivational Interviewing Trauma-Informed Care Recovery-oriented services Experience utilizing an electronic medical record system
ESSENTIAL JOB FUNCTIONS
Consumer Assessment & Treatment Conduct intake, quarterly, annual and ongoing assessments Develop person-centered treatment plans Monitor progress toward treatment goals Participate in interdisciplinary treatment planning Direct Service Delivery Deliver services consistent with recovery-oriented, person-centered, trauma-informed, and culturally responsive principles Deliver community-based behavioral health services Provide crisis intervention Provide advocacy Teach independent living skills Handle, deliver, and monitor consumer use of prescribed medications Care Coordination Coordinate with PCPs, hospitals, community agencies, and other related service providers Assist with referrals Collaborate discharges and continuity of care Documentation & Compliance Complete timely, accurate, and clinically appropriate documentation in accordance with Medicaid, CARF, agency, and regulatory requirements Complete progress notes daily Maintain documentation Complete required assessments Consumer Advocacy Assist consumers in obtaining, maintaining, and maximizing benefits, entitlements, housing, employment, transportation, and other community resources Coordinate transportation Promote consumer recovery, independence, self-advocacy, and successful community integration
