Overview Responsibilities

+ Conduct psychosocial assessments and behavioral health evaluations.

+ Develop and implement patient-centered treatment and discharge plans.

+ Provide crisis intervention, supportive counseling, and therapeutic interventions.

+ Coordinate care with psychiatrists, physicians, nurses, case managers, and community agencies.

+ Facilitate referrals to mental health, substance use disorder, and community support services.

+ Advocate for patients and families while promoting continuity of care and positive patient outcomes.

+ Maintain accurate documentation in accordance with hospital, regulatory, and compliance standards. Qualifications

+ Master's degree in Social Work, Marriage and Family Therapy, Licensed Professional Clinical Counselor, or related field.

+ License required upon hire and maintained per state regulations; otherwise, state registration and/or supervision required.

+ Current BLS certificate upon hire and maintain current.

+ Minimum one (1) year Inpatient l or Outpatient Behavioral Health experience.

+ Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.).
