Job Summary LIV Recovery is seeking a mission-driven Case Manager to support individuals as they work toward recovery, housing stability, wellness, and meaningful community connection. This role is rooted in person-centered service, dignity, respect, accountability, and hope. The Case Manager partners with clients to understand their goals, strengths, needs, preferences, culture, supports, and barriers, then helps connect them to services and resources that support long-term recovery and stability. Responsibilities:
Build respectful, trusting relationships with clients and provide strengths-based case management support. Partner with clients to develop and follow individualized, person-centered plans focused on recovery, housing stability, wellness, employment, education, and community connection. Help clients access treatment services, recovery supports, housing resources, mental health services, vocational supports, peer support, and other community resources. Provide regular one-on-one case management sessions, monitor progress, and adjust support plans in collaboration with clients and the care team. Support clients with independent living skills, tenant rights and responsibilities, budgeting, relapse prevention, and long-term housing stability. Coordinate with clinical staff, supervisors, housing providers, community partners, and other stakeholders to support continuity of care. Maintain accurate, timely, and person-centered documentation of assessments, service plans, progress notes, referrals, coordination of care, follow-up, and outcomes. Support CARF and CSTAR expectations through documentation integrity, chart review participation, service coordination, quality improvement activities, and mission-aligned service delivery. Participate in multidisciplinary team meetings, case conferences, training, audits, and continuous improvement efforts. Demonstrate consistent attendance, professional communication, confidentiality, and alignment with LIV Recovery's mission, vision, and values. Qualifications Bachelor's degree in social work or a related field preferred; four years of experience in mental health, substance use, case management, housing services, or a related field may be considered in place of a degree. Minimum of two years of experience working with individuals experiencing behavioral health needs, substance use disorders, mental health disorders, homelessness, or related barriers. Valid driver's license required. Strong verbal and written communication skills. Strong computer skills, including accurate data entry, documentation, and information retrieval. Ability to provide services with empathy, professionalism, cultural humility, and respect for client voice and choice. Knowledge of or willingness to learn CARF accreditation standards, Missouri CSTAR program expectations, behavioral health documentation requirements, client rights, confidentiality practices, and quality improvement processes. Work Setting and Physical Requirements Work may occur in diverse environmental settings. Some travel between locations is required. Must be able to sit and stand for extended periods of time. Must be able to lift 25 pounds or more. Regular and consistent attendance is essential.
