We offer a full continuum of care: Substance Abuse Comprehensive Outpatient Treatment (SACOT), Substance Abuse Intensive Outpatient Program (SAIOP), Outpatient (OP), and Mental Health Partial Hospitalization (PHP). We are committed to compassionate, evidence-based treatment and to strengthening our local community. Position Summary We're looking for a dedicated Case Manager to join our clinical team. You'll provide individualized case management to clients across our SACOT, SAIOP, OP, and Mental Health PHP programs. You'll coordinate care, remove barriers to treatment, and help clients build the stability they need for long-term recovery. You'll work closely with our Clinical Director, Operations Team, and Behavioral Health Technicians so every client has wraparound support from admission through discharge and beyond. What You'll Do Conduct comprehensive needs assessments and develop individualized service plans that reflect each client's level of care and recovery goals Coordinate referrals to community resources, including housing, transportation, employment, benefits, and social services Support smooth transitions as clients step up or down between SACOT, SAIOP, OP, and PHP Serve as a primary point of contact for clients navigating external systems such as Medicaid, DSS, housing authorities, and courts Collaborate with the clinical team on treatment planning, discharge planning, and continuity of care Maintain accurate, timely documentation in our EMR (Kipu) in line with NC Medicaid, LME/MCO, and DHSR standards Participate in treatment team meetings, case consultations, and required trainings Maintain professional boundaries and follow all

Part 2, licensure, and organizational compliance requirements What You Bring Experience in case management, social services, behavioral health, or a related field, required CADC (Certified Alcohol and Drug Counselor), required Familiarity with NC Medicaid, Vaya Health, or community behavioral health systems is a plus Strong organizational skills and close attention to documentation Ability to work well within a multidisciplinary team Reliable transportation and a valid NC driver's license Lived experience in recovery is valued and respected here

Compensation & Benefits Salary:
