Direct Care & Support:

Provide in-home and community-based interventions, including crisis management and supportive counseling to individuals with severe mental health or substance use disorders.

Clinical Collaboration:

Work with the CST Team to create and implement person-centered plans that reduce symptoms and enhance daily living.

Service Coordination:

Connect clients with community resources and participate in regular case consultations.

Documentation:

Maintain detailed records of engagements, treatment notes, and goals in compliance with regulations.

Case Management:

Monitor services and assist with medication management and psychiatric needs

Education:

Bachelor's or Master's degree.

Experience:

Minimum of 1-4 years of experience with the adult mental health population, depending on the degree level.

Skills:

Knowledge of evidenced-based models, motivational interviewing, and crisis intervention.

Environment:

Field-based, involving travel to client homes and community settings.
