Job Description

Senior Care Therapy seeks a Behavioral Health Liaison to coordinate behavioral health services for seniors in long-term care, assisted living, and community settings. In this role, you will conduct screenings and psychosocial assessments, support treatment planning, and serve as the bridge between clinicians, facility staff, residents, and families. You'll help manage referrals, facilitate on-site and telehealth services, and ensure accurate clinical documentation. Working within a collaborative, patient-centered culture, you will join an interdisciplinary team dedicated to compassion, clinical excellence, and improving seniors' quality of life. Senior Care Therapy offers flexible scheduling, training, and growth opportunities while you help enhance mental health outcomes and overall well-being for older adults. Responsibilities Serve as liaison between behavioral health providers, facility staff, residents, and families to coordinate care Conduct behavioral health screenings and psychosocial assessments for seniors in long-term care, assisted living, and community settings Support treatment planning and monitor residents' progress, communicating updates with interdisciplinary teams Provide psychoeducation to residents, families, and staff on mental health, dementia, and behavior management Facilitate referrals for psychotherapy, psychiatry, and community resources as appropriate Document assessments, interventions, and care coordination activities in accordance with clinical and regulatory standards Collaborate with nursing, social work, and therapy teams to address behavioral and emotional needs impacting daily functioning Participate in case conferences, care plan meetings, and quality improvement initiatives Support telehealth and on-site behavioral health services, ensuring smooth scheduling and follow-up Maintain compliance with HIPAA, facility policies, and professional ethical standards Required Skills Behavioral health assessment Care coordination Geriatric mental health knowledge Electronic health record (EHR) documentation Interdisciplinary team collaboration Crisis intervention and de-escalation Knowledge of long-term care/assisted living workflows Patient and family education Telehealth support and coordination Regulatory and HIPAA compliance
