Job Description

Responsible for collaborating with the medical team to develop and implement a plan of care which coordinates all of the required services. Must develop a knowledge and working relationship with outside social service agencies to ensure patients have access to all eligible program Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Bachelor's degree in Social Work, Psychology, Psychiatric Rehabilitation or mental health related discipline. Bilingual (Spanish/English) preferred. Work requires the ability to counsel and educate consumers and their families and to resolve problems that require the use of basic mental health knowledge.
