In this role, you will act as the primary point of contact helping participants navigate complex state Medicaid systems, and community resources. Your core mission is to empower participants to remain safely and independently within their homes and communities, Duties Conduct comprehensive patient assessments, including intake and social services evaluations, to develop tailored care plans. Coordinate with healthcare providers across hospital departments such as ICU, emergency medicine, pediatrics, and trauma centers to ensure appropriate care delivery. Travel within the state of New Mexico to complete assessments and intakes. (Travel incentives provided) Collaborate directly with participants, their families, and medical teams to develop, implement, and update comprehensive Individualized Service Plans (ISPs) tailored to their cognitive, physical, and emotional goals. Perform utilization review and utilization management activities to ensure compliance with health insurance policies and medical necessity criteria. Document all case management activities accurately by maintaining case files and HIPAA compliance. Communicate effectively with clients, families, and healthcare teams to provide clear information about treatment options, insurance coverage, and social services. Recognize potential risk factors or changes in functional ability, providing immediate crisis intervention, safety planning, and incident reporting as required by state law. Work closely with managed care organizations and insurance providers to facilitate approvals and resolve coverage issues efficiently. Experience/Requirements Bachelor's degree in Social Work, Psychology, Human Services, Public Health, or a closely related field. Experience working with individuals with disabilities, brain injuries, behavioral health needs, or other complex support needs. Strong understanding of person-centered planning and community-based services. Excellent communication, organization, documentation, and time-management skills. Ability to work independently while collaborating effectively with participants, families, providers, and multidisciplinary teams. Ability to maintain professional boundaries and protect confidential participant information. Valid driver's license, reliable transportation, and ability to travel within New Mexico as required.
