Duties Conduct client assessments to identify needs related to health, housing, social services, and behavioral health. Provide crisis intervention and crisis management to stabilize individuals experiencing emergencies or mental health crises. Offer addiction counseling and behavioral health support tailored to individual circumstances. Assist clients with resource identification within social services systems, including Medicaid and Medicare navigation. Work with culturally diverse communities and seniors to ensure equitable access to services and support. Utilize motivational interviewing techniques to encourage positive behavioral change and client engagement. Maintain accurate documentation through writing detailed case notes, reports, and service plans. Requirements Proven experience in social work, case management, or related fields involving client communication and resource coordination. Knowledge of crisis intervention strategies, addiction counseling, and behavioral health practices. Certification in CPR and First Aid is preferred; additional certifications such as CPR or mental health first aid are a plus. Ability to work effectively with geriatric patients, individuals with disabilities, and culturally diverse populations. Strong written communication skills for report writing and documentation purposes. Experience working with government programs like Medicaid or Medicare is highly desirable.
