In this role, you'll provide community-based behavioral health services that help youth with Serious Emotional Disturbance (SED) build the skills needed to succeed at home, in school, and in their communities. Working alongside a multidisciplinary team, you'll help youth and families achieve individualized treatment goals while promoting emotional, behavioral, social, and independent living skills. Responsibilities Provide community-based rehabilitative and supportive services to youth and families. Deliver services in home, school, and community settings. Support treatment goals through engagement, skill-building, and recovery-focused interventions. Collaborate with families, schools, healthcare providers, and community partners. Participate in crisis intervention, safety planning, and stabilization efforts. Maintain accurate documentation and comply with OMH, Medicaid, and agency standards.

Qualifications Required:

Bachelor's degree in human services, Social Work, Psychology, Counseling, Education, or a related field. 1-2 years of experience working with youth and families in behavioral health or human services. Valid New York State driver's license and reliable transportation.

Preferred:

Experience with Youth ACT, trauma-informed care, crisis intervention, CANS-NY, OMH standards, and electronic health records.
