Behavioral Health Case Management Supervisor Position Summary The Behavioral Health Case Manager is responsible for providing high-quality behavioral health interventions and case management services to patients receiving primary care and supportive services. This position helps ensure access to care, engagement and retention in services, and coordination of behavioral health and other supportive resources to promote positive health outcomes. The Behavioral Health Case Manager works as part of a multidisciplinary care team and provides culturally responsive, patient-centered services to individuals who may experience mental health, substance use, chronic health, housing, financial, or other social-service needs. Essential Duties and ResponsibilitiesClient Services and Direct Care Provide proactive behavioral health case management, including comprehensive assessment, reassessment, and development of individualized care plans. Help patients access and remain engaged in primary care, behavioral health, and other appropriate services. Communicate regularly with the multidisciplinary care team regarding patient needs, progress, barriers to care, and outcomes. Provide crisis intervention, stabilization, and brief counseling to patients as needed. Coordinate and connect patients with additional services, including specialty medical care, benefits and entitlements, long-term mental health and substance use treatment, and community-based case management. Initiate and follow up on referrals and communicate outcomes to the multidisciplinary care team. Participate in community outreach and recruitment activities designed to increase awareness of available services and reach underserved populations, including individuals with low incomes and members of communities of color. Collaboration and Service Coordination Serve as a liaison between patients and members of the multidisciplinary care team to facilitate coordination of care. Promote appropriate referrals to internal and external behavioral health providers and community-based services. Respond to telephone calls, emails, referral inquiries, and requests for information in a timely and professional manner. Participate as an active member of a multidisciplinary care team. Participate in continuous quality improvement (CQI) activities. Attend internal and external meetings, trainings, and other professional development activities as requested. Documentation and Compliance Maintain accurate, current, organized, and legible patient records. Complete and submit required forms, reports, assessments, and summaries in a timely manner. Accurately document services provided in all designated electronic and paper systems. Maintain accurate data and reporting to meet organizational, regulatory, and program requirements. Follow all applicable organizational policies and procedures, as well as relevant city, state, and federal requirements. Qualifications Advanced degree in mental health counseling, social work, or a related field required. Minimum of two (2) years of experience in a related field required. Verifiable internship and unpaid volunteer experience may be considered. LMHC or LMSW licensure required within three (3) months of hire. UAS-NY certification required within one (1) month of hire. Experience working with diverse and underserved populations strongly preferred. Experience working with individuals with serious mental illness, developmental disabilities, substance use disorders, chronic health conditions, HIV/AIDS, or other complex behavioral or medical needs is strongly preferred. Experience working with LGBTQ+ individuals, immigrants, and communities of color is a plus. Excellent written and verbal communication skills in English. Strong interpersonal and customer service skills, including the ability to respond effectively to patient and provider concerns. Strong organizational skills and the ability to manage multiple priorities. Ability to work effectively as part of a multidisciplinary team. Ability to use and adapt to a variety of electronic applications and health information systems, including Microsoft Outlook, Word, Excel, and PowerPoint, as well as electronic medical records and other clinical and case-management platforms. Ability to maintain confidentiality and exercise sound professional judgment. Commitment to providing culturally responsive, respectful, and patient-centered services. Pay:
$68,000.00 - $71,000.00 per year
