A Mental or Behavioral Health Case Manager coordinates overall plan of care for patients with mental or behavioral health needs. Oversees patient evaluation and treatment planning and monitors patient progress. Coordinates with the rest of the mental health team, including doctors and nurses, to establish these plans.
The Health Home Plus (HH+) Supervisor provides supervision to Health Home Plus Care Managers/Care Coordinators serving individuals with serious mental illness, and other complex behavioral and physical health needs. The Supervisor is responsible for ensuring that HH+ services are delivered in accordance with New York State Department of Health (NYSDOH), Office of Mental Health (OMH), Health Home, Medicaid, and agency requirements.
DETAILED RESPONSIBILITIES
¨ Provide care coordinators with support and guidance in managing their caseload ¨ Supervise Care Coordinators as they coordinate comprehensive medical and behavioral care to clients during scheduled supervision. ¨ Review member's progress notes for detail and completeness. ¨ Review and approve member's documentation in electronic health record ¨ Coordinate/case conference with other social service networks as needed ¨ Monitor program performance, productivity, quality, and compliance with HH+ requirements. ¨ Assume other duties as assigned by the Director for program success.
QUALIFICATIONS
¨ Managerial skills and knowledge ¨ Computer knowledge, including but not limited to Windows, Microsoft Office. ¨ Have the ability to work as part of a team with co-workers and with other colleagues as necessary. ¨ Cultural sensitivity and the ability to relate/work with diverse groups, community agencies, schools, and the general public. ¨ Excellent verbal and written communication skills, including strong interpersonal and organizational skills, focusing on detail. ¨ Ability to follow directions and communicate well (orally and in writing)
EDUCATION/TRAINING/EXPERIENCE
¨ Bachelor's degree in appropriate field (Social Work, Human Services, Psychology, etc.) and 3+ years' experience providing Health Home, or equivalent, SMI/HARP/SED services to individuals Or ¨ Master's degree in appropriate field (Social Work, Human Services, Psychology, etc.) and minimum 2+ years' experience providing Health Home, or equivalent, SMI/HARP/SED services to individuals
