Responsibilities:
Provides professional comprehensive care aligned with ICARE values to patients of the URMC / Strong Ties Assertive Community Treatment (ACT) teams. Collaborates with health, behavioral health and social service providers and is responsible for assessing patient's needs, developing and managing care plans with patients enrolled in ACT services. ACT serves the most complex, high utilizing patients that need comprehensive community treatment and care management services. Services include, but are not limited to: care coordination, health promotion, patient and family support, referral to community and social supports, crisis intervention, and family support. Additional services include psychosocial rehabilitation services to assist individuals manage and resolve symptoms of mental illness in school or workplace settings and maintain functional skills necessary to maintain employment or attain educational goals. Per ACT guidelines, Vocational Specialists assess the effect of the individual's mental health on employment and implement an employment strategy to enable individuals to get and keep a job. Vocational specialists help individuals seeking employment, to find the best match for their work interests and skills and preferred work environment.ESSENTIAL FUNCTIONS
Under general direction and with considerable independence, works with URMC / Strong Ties ACT team patients. Visits patients in their community setting to assess their progress toward meeting the goals of the care plan and to monitor compliance with the treatment plan. Teaches, guides, and assists patients and family members to insure their clear understanding of the patient's needs and expectations of the specific care plan. Makes recommendations to mental health professionals regarding treatment plans. Coordinates patient outreach (in person, primary care settings, behavioral health clinics, community settings including patient homes, jail, hospitals, shelters, etc.), management activities and interactions focused on locating, connecting and retaining patients in ACT treatment. Documents patient outreach and services in electronic medical record system and ensures that all documentation is timely, accurate and in compliance with Hospital policies. Conducts assessments for enrollees identifying service needs that contribute to developing a patient centered care plan. Carries out strategies for implementation of treatment plan by: establishing contact with various service providers and family members; assuring appropriate utilization of life support services in the community; and takes initiative for suggesting appropriate community services that would enable the patient to function at his/her optimal level in the community. Knowledgeable of models of supported employment/Individual Placement and Support (IPS). Knowledge of psychosocial rehabilitative services Assertive Community Treatment (ACT) Teams Guidelines. Demonstrates the ability to complete a vocational assessment to determine mental health needs in the workplace or education (e.g., career profile). Demonstrates skills development related to choosing, securing, and maintaining employment and educational goals. Assists clients in maintaining themselves in the community, including finding and furnishing apartments, setting up financial budgets, applying for entitlements, and learning skills of daily living. Transports clients as needed to appointments and assists clients in moving or shopping. Ensures that Hospital and Department policies and procedures are implemented regarding professional practice, patient care, and disposition training. Subscribes to the Department's goal of creating a culturally competent environment by treating patients/families, trainees, research subjects and co-workers in a sensitive and caring manner with appropriate attention to cultural differences. Participates in available cultural competence events and trainings. Participates on a rotational basis in on-call group during evening/weekend hours. Responds, as necessary, after hours to psychiatric crises in the community. Other duties as assigned.MINIMUM EDUCATION & EXPERIENCE
Bachelors Degree in an appropriate human services field and; or an equivalent combination of education and experience required. 2-3 years of experience providing direct services to individuals diagnosed with serious mental illness, intellectual/developmental disabilities, alcoholism/substance abuse required; or experience effectively linking individuals with services that address social determinants of health requiredKNOWLEDGE, SKILLS AND ABILITIES NYS
