Job Summary We are seeking a passionate and dedicated Peer Recovery Support Specialist / SUD (Substance Use Disorder) Case Manager to join our dynamic healthcare team. Responsibilities Provide peer support and mentorship to individuals with substance use disorders, fostering hope and motivation throughout their recovery journey Conduct comprehensive patient assessments, including intake evaluations and diagnostic evaluations, to determine needs and develop personalized care plans Coordinate discharge planning and connect clients with community resources such as mental health services, social work support, and outpatient programs Manage electronic health records (EHR) accurately and securely in compliance with HIPAA regulations, documenting all interactions and care activities Collaborate with multidisciplinary teams—including Clinical staff experienced in Substance Use Disorder medicine, trauma informed care, and behavioral health—to ensure seamless patient care transitions Assist with utilization management and case management activities to optimize treatment plans and resource utilization Work effectively with diverse populations, including individuals with Substance Use Disorders and mental health conditions, and those from various cultural backgrounds Skills Experience working within clinical and supportive housing settings such as Level I, Level II, and Level III Housing and Outpatient Clinics Knowledge of medical terminology, physiology, diagnostic evaluation, and clinical documentation Strong understanding of addiction counseling principles and working with individuals with substance use disorders or mental health conditions Proficiency in EMR/EHR systems and familiarity with HIPAA compliance standards for patient privacy Excellent client communication skills coupled with the ability to work effectively with families and multidisciplinary teams Ability to prioritize tasks efficiently in fast-paced settings such as inpatient units or outpatient clinics Experience working with individuals with disabilities is a plus; familiarity with community resource coordination enhances service delivery Basic life support (BLS) certification; CPR certification is required prior to or immediately after hire—training available if needed Join us in transforming lives through compassionate care! Monday-Friday - 4pm-12am 40 Hours per week Saturday-Sunday - 8am-8pm 24 hours per week Full time and Part time available

Pay:

$17.00 - $21.00 per hour
