Oversees patient evaluation and treatment planning and monitors patient progress. Coordinates with the rest of the mental health team, including doctors and nurses, to establish these plans.

Job Description

Community mental health program looking for a Case Manager for individuals in the Warren-Youngstown area. Flexible schedule. Work on helping connect individuals with community resources and provide advocacy and support services. Will need to develop treatment plans and monitor ongoing needs, as well as be available for clients and families. May need to attend appointments and meetings. Strong verbal and written communication skills required with a desire to help and support others. Knowledge of area resources is a plus. Requires a bachelor's degree in a mental health field. Starts at part time but will increase to full time as caseload builds. Health insurance option for those working 30+ hours per week.

$25.00 per hour
