This position requires at least one year experience working with high-risk populations. This position will involve providing case management and rehabilitation services to high-risk custody youth in a 9 bed youth shelter in Norman, OK.
Duties will include:
1. Plan and administer treatment identified in the individualized service plan (ISP) which will include individual or group rehabilitative services and/or case management services to include living skills enhancement and social skills enhancement. 2. Document all services in accordance with Community Works policy and procedures and submit in a timely fashion. Ensure compliance with all contractual and accreditation requirements, as applicable. 3.Collaborate with other professionals, family, and agency clinical staff to ensure consistency and timely completion of goals identified in the ISP. 4. Maintain regular contact with other agencies/professionals and family regarding client progress via phone calls, letters, emails and/or monthly reporting. 5. Attend agency required meetings; i.e., staff meetings, treatment team meetings, IEP meetings. 6. Collaborate with assigned therapist to provide prescribed treatment for each student; provide behavior re-direction to residents.

$14.00 - $15.00 per hour You must meet one of these minimum requirements: bachelor or master degree in a behavioral health-related field including but not limited to, psychology, sociology, criminal justice, school guidance and counseling, social work, recreational therapy, occupational therapy, family studies. OR current certification as a Behavioral Health Case Manager (1 or 2) from the Oklahoma Department of Mental Health and Substance Abuse (ODMHSAS)
