Job Description

Responsibility:
Under the direct supervision of and accountable to the Clinical Supervisor, the Mental Health Case Manager provides community-based case management, care coordination, and skill-building services to individuals and families receiving mental health services. This role supports individuals in accessing needed services, overcoming barriers to care, and improving overall stability, wellness, and functioning. The Mental Health Case Manager carries responsibility for fulfilling the administrative and clerical duties described below.
Duties:
Provide case management, care coordination, and skill-building interventions aligned with individual treatment goals Support individuals and families in accessing mental health, medical, housing, educational, and social service resources Coordinate care with behavioral health providers, primary care providers, schools, and community agencies Assist individuals in organizing, attending, and following up on appointments Advocate for individuals and families within service systems Conduct community-based and home visits as appropriate Assist individuals in identifying and resolving barriers to care Support development of self-management, coping, and daily living skills Provide culturally responsive services to diverse and underserved populations Document services accurately and timely in the electronic medical record and required tracking systems Track caseload activity, service outcomes, and required metrics Participate as a member of the multidisciplinary treatment team Attend scheduled meetings, trainings, and supervision as required Complete other administrative or program-related tasks as assigned by the
Clinical Supervisor or Associate Executive Director Competencies:

Physical Requirements:
Prolonged periods of sitting and working on a computer Ability to walk, stand, and navigate stairs during community and home visits Ability to use hands and fingers for documentation and office tasks Ability to visually assess environments and engage in community-based work
Requirements:
Bachelor's degree in behavioral science, social services, or related field, or a combination of at least three (3) years of relevant education, training, or experience Eligibility as a Qualified Mental Health Associate (QMHA) under applicable Oregon Administrative Rules Basic knowledge of motivational interviewing, harm reduction, and stages of change Knowledge of community resources including mental health, healthcare, housing, and social services Valid driver's license, acceptable driving record, and insured vehicle Ability to work effectively with individuals from diverse cultural and socioeconomic backgrounds
