Role:
As a part-time Social Work/Care Companion , you will visit designated facilities, conduct assessments, and implement short-term, solution-focused therapy and supportive interventions to address immediate concerns. Your role is ideal for professionals who want to make a direct impact on the lives of older adults, helping them feel heard, supported, and emotionally connected within their environment.Responsibilities:
Visit designated facilities in accordance with the schedule set with your supervisor Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns Document visit notes and maintain detailed records of interactions and interventions after each visit Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatricNP Qualifications:
Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings Strong communication, documentation, and relationship-building skillsEducation:
