Job Description
Case Manager's job is to help individuals navigate and access resources within the healthcare or social service system, ensuring they receive the best possible care and support. They assess client needs, develop individualized care plans, coordinate services, advocate for clients, and monitor progress to achieve optimal outcomes. Responsibilities Provide crisis intervention and addiction/mental health counseling to support clients in distress. Conduct motivational interviewing to empower clients in their recovery journey. Collaborate with community resources to enhance client services and support systems. Monitor client progress and adjust care plans as necessary to achieve optimal outcomes. Facilitate clinical groups daily Conduct individual therapy sessions as needed
