Recovery Support Outreach Case Manager TriSource is seeking a dedicated Recovery Support Outreach Case Manager . In this role, you will provide individualized, non-clinical case management and outreach support to individuals recovering from substance use and/or mental health challenges. You will help clients gain the skills and resources necessary to initiate, maintain, and sustain long-term recovery, stability, and well-being.

Monday-Friday 8:30pm-5pm

Pay Rate:

$22-$23/

Hour Qualifications & Skills Education:

Bachelor's degree in social work, psychology, counseling, or a related field is required.

Experience:

Minimum of two years of experience in case management, behavioral health, or recovery support services. Direct experience working with chemically dependent populations is valued.

Certifications:

Certification as a Certified Recovery Specialist (CRS), Certified Peer Specialist (CPS), or equivalent credential is required. Lived experience with substance use or mental health recovery is preferred. Key Responsibilities Conduct comprehensive assessments of clients' strengths, needs, and goals within the first 30 days of enrollment, with updates completed every 6 months. Develop, implement, and monitor individualized recovery support plans and adjust them as needed. Collaborate closely with the Certified Peer Recovery Specialist, Recovery Manager, and other New Pathways Project team members. Coordinate linkages and advocate for participants to secure physical health services, entitlements, educational, employment, and vocational services. Engage in community outreach and external field follow-up with participants, requiring a minimum of 2 days in the field. Educate clients on harm reduction strategies, coping skills, and wellness practices, and facilitate support groups or workshops as needed. Maintain accurate, timely documentation that complies with organizational and regulatory requirements. Advocate for clients across healthcare, legal, and social service systems, and follow up to re-engage individuals into program or treatment services.
