Job Description

Description PrimeCare Medical is currently looking for a Case Manager to work in the medical department at the Chester County Prison in West Chester, PA. This is a Part Time Monday - Friday position, 20 hours per week, during normal business hours. (8:00am-4:30pm).
Job Responsibilities:
The case manager is responsible for assisting in the coordination and implementation of the behavior health program.
Other duties include:
Responsible for the clinical monitoring, coordination, and implementation of the behavioral health program in accordance with
NCCHC/ACA/MCCS
Standards. Actively assist with the detection and prevention of inmate/patient suicide by reporting concerns to supervisory staff. Assist in the coordination of the admission and placement of certified inmates/patients in forensic and community behavioral health facilities. Initiate the paperwork required for the inmates/patient's admission. Document clinical services provided in the electronic medical record in a complete, accurate and timely manner. Monitor inmates/patients to interventions and report any concerns to supervisory staff. Assist patients, particularly those identified as seriously mentally ill, in developing a discharge plan to continue therapeutic services in the community. Coordinate with community providers to assist inmates/patients in identifying resources that are available to them post-release. Obtain releases from patients to request and obtain records from patient's previous treatment providers. Serve as a member of various committees within the medical unit and facility, as necessary. Participate in all in-service education programs according to the annual in-service calendar. Maintain confidentiality at ALL times. Any/all other duties and responsibilities as assigned or delegated.
Benefits:
Salaries are negotiable and commensurate based on experience. Hourly rate range $22.00 - $25.00 Part Time PTO package provided and accrued at the rate of 1.0 hour for every 30.0 hours of straight time worked. Max number of PTO hours that may be accrued is 64.0 hours. Requirements Bachelor's Degree in Social Work, Psychology, Counseling, or other related Human Services degrees with a concentration in mental health course work preferred. A minimum of one (1) year clinical experience is preferred.
