We are seeking a compassionate and dedicated Case Manager to join our team and provide essential support and guidance to individuals facing various challenges. The ideal candidate will have experience in behavioral health, social work, or related fields, and possess strong skills in crisis intervention, addiction counseling, and crisis management. This role involves coordinating services, developing individualized plans, and advocating for clients to promote their well-being and independence. The position offers an opportunity to make a meaningful difference in the lives of those in need while working within a collaborative and supportive environment. Responsibilities Conduct comprehensive assessments to identify clients' needs related to behavioral health, child welfare, or housing programs such as Section 8 and Medicare Develop personalized care plans and coordinate services across multiple agencies and community resources Provide crisis intervention and management to stabilize clients during emergencies or high-stress situations Utilize motivational interviewing techniques to encourage positive behavioral changes and treatment adherence Offer addiction counseling support and facilitate access to treatment programs Supervise and mentor clients, including those involved in workforce development or childcare initiatives Assist clients with navigating complex systems such as Medicaid, Medicare, or child welfare services Monitor client progress regularly and adjust care plans accordingly Maintain detailed documentation of client interactions, service plans, and progress reports in compliance with agency policies Requirements Proven experience in crisis intervention, behavioral health, social work, or related fields Strong interpersonal skills with the ability to motivate and mentor diverse populations Mentoring experience is preferred Excellent communication skills for effective advocacy and client engagement Ability to handle sensitive situations with empathy and professionalism Relevant certifications or licenses in social work or counseling are a plus Ability to work independently as well as part of a multidisciplinary team New Mexico resident This position is integral to supporting vulnerable populations through compassionate care and resource coordination.
