A Mental or Behavioral Health Case Manager coordinates overall plan of care for patients with mental or behavioral health needs. Oversees patient evaluation and treatment planning and monitors patient progress. Coordinates with the rest of the mental health team, including doctors and nurses, to establish these plans.

IHN provides comprehensive, person-centered case management and care coordination for adults with complex medical, behavioral health, cognitive, aging, disability, and psychosocial needs. Compensation:
$35-$60/hour , based on education, professional credentials, directly relevant experience, and the complexity of work the candidate is qualified to perform. Highly experienced senior-level candidates may qualify for compensation up to $70/hour based on advanced education, applicable professional licensure, and demonstrate complex case-management or clinical experience.
What You'll Do:
Conduct comprehensive client assessments in home and community settings Develop, implement, and monitor individualized plans of care Coordinate medical, behavioral health, residential, social service, and community resources Collaborate professionally with physicians, healthcare providers, families, guardians, facilities, and interdisciplinary teams Attend medical appointments, care conferences, discharge-planning meetings, and other client-related meetings as appropriate (escorting of client(s) may be necessary) Monitor changes in client health, behavior, safety, placement, and overall well-being and appropriately escalate concerns Advocate for clients and assist in resolving barriers to appropriate care and services Maintain accurate, professional, and timely case documentation

Qualified candidates should have:
A bachelor's degree or higher in psychology, social work, human development, nursing, healthcare, behavioral health, or another relevant discipline At least 2 years of directly relevant professional experience Experience working with adults with complex medical, behavioral health, cognitive, disability, aging, or psychosocial needs Strong assessment, problem-solving, advocacy, and care-coordination skills Excellent written and verbal communication Sound professional judgment and appropriate professional boundaries Strong organization, time management, documentation, and follow-through The ability to independently prioritize multiple client(s) needs while collaborating effectively with the IHN team
Particularly Valuable Experience:
Experience in any of the following is highly desirable: Complex case or care management Mental and behavioral health Substance use disorders Older adults and dementia Adults with disabilities Long-term care and residential placement Hospital discharge and transitional care Guardianship or professional fiduciary environments Motivational interviewing Professional licensure in nursing, social work, behavioral health, or a related discipline, as well as CCM or comparable professional certification , is highly valued but is not required for all Case Manager positions.
