Job Description
This is a Part-Time opportunity! Anticipated 15-25 hours a week within normal business hours. Flexible schedule! We're looking for a Case Manager (Member Engagement Specialist) to play an essential role in supporting individuals through their recovery journey. You'll be responsible for the initial engagement and enrollment of eligible health plan members within a treatment facility or community-based setting, helping them understand the valuable services Wayspring offers. You'll continue to support and engage members throughout their program, ensuring they have the support and resources they need to succeed. This is your chance to be a difference-maker! We're looking for someone who is not just a professional, but a passionate advocate for the wellbeing of those in your community. The ideal candidate is someone with strong communication skills, experience in discharge planning, social work, peer recovery, behavioral health, mental health, or the substance use disorder space, and a genuine desire to see others succeed. This role requires regular daytime travel to meet members onsite at facilities in and around Rockford, IL and surrounding areas

Ideal candidates have experience:
Conducting street-based or mobile outreach and engaging individuals with substance use or behavioral health challenges Supporting clients through recovery services, harm reduction programs, or case management Working within addiction recovery, harm reduction, mental health, or community-based programs Building trust with individuals facing complex social, behavioral health, or addiction-related barriers Collaborating with nonprofits, residential treatment centers, public health agencies, or grassroots organizations Our goal is to foster a workplace where everyone feels a true sense of belonging, is supported, and empowered to thrive.
