Job Description

Position Overview New Directions Community Outreach Services is seeking a motivated, organized, and results-driven Case Manager - Mental Health to join our growing behavioral health team. This is a leadership-focused position for a professional who can effectively balance client care coordination with staff oversight, productivity management, documentation compliance, quality assurance, and community referral development . The Case Manager will provide day-to-day oversight and support to assigned QMHPs and direct-care staff, helping ensure that services are delivered consistently, appropriately, and in accordance with client treatment plans, authorized hours, Medicaid requirements, and agency standards. The successful candidate will serve as an important link between clinical staff, clients and families, administration, and community referral partners . This individual must be comfortable holding staff accountable, identifying performance concerns, coaching employees toward improvement, and taking initiative to help grow the agency through professional community outreach and development of viable client referrals. This position is ideal for a behavioral health professional who enjoys leadership, problem-solving, staff development, community networking, and making a measurable difference in client outcomes . Key ResponsibilitiesClinical Leadership & Case Management Provide case management and service-coordination support to assigned clients and families. Assist with identifying client strengths, needs, barriers, and appropriate behavioral health resources. Monitor client progress and ensure services remain aligned with individualized treatment goals and service plans. Collaborate with QMHPs, therapists, caregivers, clinical leadership, schools, healthcare providers, DSS, probation, CSBs, and other community partners. Participate in case staffing, treatment planning, service coordination, and discharge planning. Identify emerging client needs, service barriers, and safety concerns and communicate them appropriately to leadership. QMHP & Direct-Care Staff Supervision A major responsibility of this position is providing day-to-day oversight and accountability for assigned QMHPs and direct-care staff . Monitor assigned staff to ensure clients are receiving services in accordance with authorized hours and treatment plans. Review staff schedules, caseloads, and service utilization to promote appropriate distribution and maximization of authorized hours. Monitor staff attendance, service delivery, productivity, and overall performance. Ensure staff are meeting agency expectations related to professionalism, client engagement, communication, and service delivery. Provide ongoing coaching, feedback, accountability, and performance guidance. Address concerns related to missed sessions, inconsistent service delivery, poor communication, productivity, or failure to meet documentation requirements. Collaborate with agency leadership regarding performance concerns and assist with the implementation of corrective or performance-improvement strategies. Help staff identify barriers to successful service delivery and develop practical solutions. Support new staff as they transition into their roles and reinforce agency expectations and procedures. Documentation, Quality Assurance & Compliance Monitor clinical documentation for accuracy, completeness, timeliness, and medical necessity. Conduct routine reviews of staff documentation and provide corrective feedback when needed. Monitor completion of progress notes, assessments, treatment plans, monthly reports, and other required documentation. Ensure documentation and service delivery are consistent with Medicaid, DBHDS, licensing, HIPAA, and agency requirements . Identify documentation trends or compliance concerns and communicate them to leadership. Assist with preparing assigned cases for internal audits, reviews, and external quality-assurance activities. Maintain accurate and current client and staff records. Productivity & Service Utilization Monitor staff productivity and client service utilization on an ongoing basis. Ensure authorized service hours are being appropriately scheduled and utilized. Identify underutilization, missed service hours, scheduling barriers, and potential gaps in service delivery. Work proactively with staff to correct productivity concerns before they negatively affect client care or authorization utilization. Communicate significant productivity concerns to leadership. Community Outreach & Referral Development Referral development is an essential responsibility of this position. The Case Manager is expected to actively participate in marketing, community outreach, relationship development, and solicitation of viable new client referrals .
Responsibilities include:
Develop and maintain professional relationships with schools, school counselors, school social workers, DSS/CPS/APS, juvenile and adult probation, CSBs, healthcare providers, hospitals, therapists, physicians, CSA/FAPT offices, courts, community organizations, and other referral sources. Conduct scheduled outreach calls, emails, visits, presentations, and follow-up communications with potential referral partners. Educate community partners about agency services, eligibility requirements, referral procedures, and areas of service. Identify potential referral opportunities and obtain appropriate points of contact. Follow up consistently with referral sources to develop and maintain professional relationships. Assist with obtaining and screening viable referrals based on agency services and client eligibility. Maintain accurate referral and outreach records. Provide leadership with updates regarding outreach activity, referral opportunities, community partnerships, and potential program-growth opportunities. Represent the agency professionally and positively throughout the community. The successful candidate should be comfortable with proactive business development and understand that building referral relationships is an ongoing component of the position. Team Collaboration & Agency Leadership Participate in staff meetings, case consultations, training, supervision, and quality-assurance activities. Serve as a communication link between direct-care staff, clinical leadership, administration, clients, families, and external providers. Promote a culture of accountability, professionalism, teamwork, and client-centered service. Assist leadership with implementation of agency policies, procedures, and initiatives. Provide constructive feedback and help staff maintain high standards of service delivery. Complete additional responsibilities assigned by agency leadership. Someone who does not wait to be told what needs to be done and is comfortable identifying problems and developing solutions. Minimum & Preferred Qualifications Bachelor's or master's degree in Social Work, Counseling, Psychology, Human Services, Behavioral Health, or a related field . Relevant experience in community mental health, behavioral health, case management, IIHS, MHSS, PSR, outpatient services, or related human services. QMHP certification or applicable LMHP credential preferred/required based on position responsibilities and applicable regulations. Previous experience supervising, coordinating, coaching, or managing behavioral health staff strongly preferred. Working knowledge of Medicaid behavioral health services and documentation requirements. Experience with electronic health records and clinical documentation systems. Strong written, verbal, organizational, and interpersonal communication skills. Ability to effectively communicate with children, adolescents, adults, families, professionals, and community partners. Strong problem-solving, decision-making, and conflict-resolution abilities. Ability to independently manage multiple priorities and meet established deadlines. Demonstrated ability to develop professional relationships and generate community-based referral opportunities. Experience with CSA/FAPT, DSS, probation, schools, CSBs, or other Virginia behavioral health systems strongly preferred. Spanish-speaking candidates are encouraged to apply. Support agency growth while maintaining a strong focus on quality client care and regulatory compliance.
Pay:
$23.00 - $25.00 per hour Expected hours: 10.0 - 20.0 per week
