This position offers an exciting opportunity to coordinate patient care, facilitate discharge planning, and connect individuals with essential community resources. You will work closely with multidisciplinary teams in diverse clinical settings, including inpatient units, outpatient clinics, and community-based programs, ensuring that each patient receives personalized, high-quality care. Your expertise will help improve patient outcomes and foster recovery through compassionate support and strategic case management. Duties Conduct thorough patient assessments to determine mental health needs, including diagnostic evaluations and behavioral health screenings. Develop individualized treatment plans in collaboration with healthcare providers, patients, and families to address mental health conditions and substance use disorders. Coordinate discharge planning by arranging post-acute care services, community resources, and follow-up appointments to ensure seamless transitions. Manage electronic health records (EHR) accurately and securely in compliance with HIPAA regulations while documenting patient interactions and treatment progress. Provide clinical case management by monitoring patient progress, adjusting care plans as needed, and advocating for patient needs within hospital or community settings. Engage in utilization management to ensure appropriate level of care while optimizing resource allocation. Facilitate communication between patients, families, healthcare teams, and community agencies to promote understanding and support for recovery goals. Experience Demonstrated background in behavioral health counseling, mental health assessments, or social work related to inpatient or outpatient psychiatric services. Experience working with individuals across diverse cultural backgrounds, including children and those with disabilities or substance use disorders. Knowledge of medical terminology, physiology relevant to mental health conditions, and clinical documentation standards using EMR/EHR systems. Strong skills in patient service delivery, client communication, task prioritization, and working effectively within multidisciplinary teams. Certifications such as Basic Life Support (BLS), CPR, and relevant licensure as a Qualified Mental Health Professional are required; additional experience in addiction counseling or managed care is a plus. Familiarity with community resource coordination in health services social work enhances your ability to support holistic patient recovery.
