We are seeking experienced, compassionate, and self-motivated professionals to become part of our founding team. This is a preliminary hiring opportunity for professionals who are passionate about improving access to behavioral health, medical, and community-based services while helping individuals achieve greater independence and quality of life. Job Summary As a Medicaid Targeted Case Manager, you will coordinate comprehensive, person-centered care for Medicaid members by assessing needs, developing individualized service plans, connecting members with healthcare providers and community resources, monitoring progress, and advocating for access to medically necessary services. You will collaborate with behavioral health providers, medical professionals, hospitals, schools, community organizations, and social service agencies to ensure members receive coordinated, high-quality care while remaining compliant with South Carolina Medicaid standards. Primary Responsibilities Conduct comprehensive intake assessments to determine client strengths, needs, and eligibility for Medicaid Targeted Case Management. Develop individualized, person-centered service plans that address behavioral health, medical, housing, educational, vocational, and social support needs. Coordinate services with healthcare providers, hospitals, behavioral health agencies, schools, and community organizations. Assist members during care transitions, including hospital discharge planning and linkage to ongoing community services. Educate members and families regarding available Medicaid benefits, treatment options, community resources, and health-related services. Monitor member progress through routine contacts and service plan updates. Advocate for members to obtain medically necessary services and remove barriers to care. Maintain accurate, timely documentation within the agency's electronic health record (EHR) system while ensuring HIPAA compliance. Collaborate with multidisciplinary teams to improve outcomes and continuity of care. Participate in agency meetings, required trainings, quality improvement initiatives, and Medicaid compliance activities.
Minimum Qualifications Applicants must possess:
Bachelor's degree in: Psychology Social Work Counseling Nursing Or another Medicaid-approved human services field Minimum of one (1) year of documented case management experience Strong communication, organizational, documentation, and time management skills Ability to work independently while managing an assigned caseload Reliable transportation and willingness to travel throughout assigned service areas Ability to pass all required background checks and Medicaid credentialing requirements Preferred Qualifications Preference will be given to candidates with experience in: Medicaid Targeted Case Management (MTCM) Behavioral Health Case Management Community Mental Health Managed Care Care Coordination Electronic Health Records (EHR/EMR) Hospital discharge planning Working with children, adolescents, adults, or individuals with serious mental illness Bilingual communication skills

Medicaid Targeted Case Managers earn 50% of collected Medicaid reimbursement for completed, billable, and compliant MTCM services. Compensation is based on timely documentation, claim approval, and successful Medicaid reimbursement.
