About the Blended Case Management Role As a Blended Case Manager within our Mental Health service line team, you'll play a vital role in coordinating and advocating for the mental health needs of the individuals served. This position is ideal for someone who is compassionate, resourceful, and thrives in both independent and collaborative work environments and thrives in fast-paced environments where collaboration matters.
Starting Rate:
Based on education, experience and qualifications. Blended Case Manager Core Responsibilities Conducting initial assessments to determine the needs of clients Coordinating with individuals served, their families, and team members to develop treatment plans Connect individuals with appropriate mental health services and resources that can facilitate well-being Regularly monitoring goal progress and providing onboarding support, guidance, and encouragement Respond promptly and effectively to any mental health crises that may arise, ensuring their safety and well-being Serve as an advocate for individuals by ensuring their rights are upheld, their voices are heard, and they have access to resources and services to improve their overall quality of life What You Bring Bachelor's Degree in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation, or related field 12 credit hours in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation, or a related field AND 2 years experience Strong interpersonal and communication skills Comfortable using technology and working both independently and as part of a team A valid driver's license and reliable transportation, and ability to meet standard background and clearance requirements Who Thrives in This Role We've found that people who are: Empathetic and adaptable Organized and detail-oriented Collaborative and communicative Self-motivated and reliable ...... to thrive in this role. Office hours are Monday through Friday, 8:30 AM to 4:30 PM. Mental Health Case Managers have on-call responsibilities.
