Provides peer-led, post-crisis follow-up services for individuals referred through 988, mobile crisis, emergency departments, EMS, law enforcement, and behavioral health providers. Uses lived experience, recovery support, and resource navigation to improve engagement, continuity of care, and recovery outcomes.

Key Responsibilities:

Initiate outreach within 24-48 hours; provide support for up to 90 days; offer emotional support, recovery coaching, safety planning, appointment assistance, and service coordination; connect participants to behavioral health, substance use, housing, transportation, employment, and community resources; conduct structured follow-up for high-risk individuals; collaborate with community partners; document services and outcomes.

Qualifications:

High school diploma or GED required; associate or bachelor's degree preferred. Certified Peer Recovery Specialist credential or ability to obtain within first 90 days required. Lived experience with mental health or substance use recovery. Peer support, case management, crisis services, or resource navigation experience preferred.

Knowledge & Skills:

Strong communication, empathy, advocacy, organization, computer skills, knowledge of local resources, and ability to engage individuals in a recovery-oriented, trauma-informed manner.

Pay:

$41,600.00 per year
