Duties Conduct thorough patient assessments, including medical history review, physiological evaluation, and psychosocial needs analysis. Develop individualized care plans that align with clinical guidelines and patient preferences, emphasizing acute care, inpatient, outpatient, and post-acute settings. Coordinate with healthcare providers across hospital departments such as ICU, emergency medicine, pediatrics, and behavioral health to ensure optimal patient care. Manage electronic health records (EHR) efficiently while maintaining strict adherence to HIPAA regulations to protect patient confidentiality. Facilitate discharge planning by connecting patients with community resources, social services, mental health support, addiction counseling, and home health agencies. Prioritize tasks effectively to handle multiple cases involving complex medical conditions or disabilities while ensuring timely interventions. Communicate clearly with patients and their families to explain treatment options, provide emotional support, and foster understanding of care plans. Requirements Proven hospital experience with exposure to Level I Trauma Centers or Level II trauma centers is highly desirable. Strong background in acute care environments such as ICU, emergency medicine, or hospital medicine. Experience working with children and individuals with disabilities or substance use disorders is preferred. Knowledge of medical terminology, physiology, diagnostic evaluation techniques, and clinical case management principles. Familiarity with electronic health records (EHR) systems and healthcare compliance standards including HIPAA. Certification in Basic Life Support (BLS) and CPR is required; additional certifications in addiction counseling or mental health counseling are a plus. Excellent client communication skills with the ability to work effectively with diverse populations from various cultural backgrounds. Demonstrated ability to prioritize tasks efficiently while managing multiple cases in inpatient or outpatient settings.
