FOR FIRST RESPONDERS
We are seeking a dedicated and compassionate Substance Use Disorder (SUD) Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate comprehensive care plans for individuals battling substance use disorders, ensuring they receive the appropriate treatment, support, and resources needed for recovery. Your expertise will help bridge the gap between clinical services and community resources, fostering positive outcomes for diverse patient populations. This position offers an exciting opportunity to make a meaningful difference in people's lives through proactive case management, patient advocacy, and collaborative care. Responsibilities Conduct thorough patient assessments to evaluate substance use history, mental health status, and overall physical health, utilizing clinical case management techniques. Develop individualized treatment plans in collaboration with multidisciplinary teams, including physicians, social workers, and addiction specialists. Coordinate inpatient and outpatient services, including discharge planning and post-acute care arrangements, to ensure seamless transitions across care settings. Provide patient education on substance use disorders, recovery strategies, medication management, and community resources to promote sustained sobriety. Monitor patient progress through regular follow-ups, documentation in electronic health records (EHR), and utilization management processes. Facilitate communication with patients' families and support systems to foster a strong network of care and understanding. Maintain compliance with HIPAA regulations and uphold confidentiality standards while managing sensitive medical records and documentation. Qualifications Proven experience working in hospital environments with acute care settings such as ICU, emergency medicine, or Level I/II trauma centers. Strong background in behavioral health, mental health counseling, addiction counseling, or social work related to substance use disorders. Familiarity with hospital medicine protocols, discharge planning procedures, and post-acute care coordination. Knowledge of medical terminology, physiology related to addiction recovery, and diagnostic evaluation techniques. Proficiency in electronic health records (EHR) systems and understanding of HIPAA compliance requirements. Experience working with diverse populations including children, individuals with disabilities or mental health conditions, and those from various cultural backgrounds. Ability to prioritize tasks efficiently while maintaining excellent patient service and communication skills.
