We are seeking a dedicated and compassionate Substance Use Disorder (SUD) Case Manager to join our dynamic healthcare team. Responsibilities Conduct thorough patient assessments to evaluate substance use history, mental health status, and overall physical health, utilizing clinical case management techniques. Develop individualized treatment plans in collaboration with multidisciplinary teams, including physicians, social workers, and addiction specialists. Coordinate inpatient and outpatient services, including discharge planning and post-acute care arrangements, to ensure seamless transitions across care settings. Provide patient education on substance use disorders, recovery strategies, medication management, and community resources to promote sustained sobriety. Monitor patient progress through regular follow-ups, documentation in electronic health records (EHR), and utilization management processes. Qualifications Proven experience working in hospital environments with acute care settings such as ICU, emergency medicine, or Level I/II trauma centers. Strong background in behavioral health, mental health counseling, addiction counseling, or social work related to substance use disorders. Familiarity with hospital medicine protocols, discharge planning procedures, and post-acute care coordination. Knowledge of medical terminology, physiology related to addiction recovery, and diagnostic evaluation techniques. Proficiency in electronic health records (EHR) systems and understanding of HIPAA compliance requirements. Experience working with diverse populations including children, individuals with disabilities or mental health conditions, and those from various cultural backgrounds. Ability to prioritize tasks efficiently while maintaining excellent patient service and communication skills.
