Position Summary The Targeted Case Manager / Community Navigator works directly with individuals and families in the community to identify needs, develop service plans, coordinate resources, and support successful outcomes. This position offers flexibility and autonomy while making a meaningful impact in the lives of those served. Responsibilities Conduct client assessments and identify barriers to care. Develop and implement individualized service plans. Coordinate housing, healthcare, behavioral health, transportation, educational, employment, and community resources. Maintain regular contact with clients, families, providers, and community partners. Document all services accurately and timely within the agency's electronic health record system. Participate in supervision and training activities. Maintain compliance with Medicaid and agency documentation standards. Qualifications Bachelor's degree in Psychology, Social Work, Human Services, Criminal Justice, Public Health, or related field. Certified Behavioral Health Case Manager (CBHCM) or eligibility to pursue certification preferred. Experience working with vulnerable populations, behavioral health, social services, or community-based programs preferred. Strong interpersonal, organizational, and problem-solving skills. Reliable transportation and ability to conduct community visits. Ability to work independently and manage a flexible schedule.
