A Mental or Behavioral Health Case Manager coordinates overall plan of care for patients with mental or behavioral health needs. Oversees patient evaluation and treatment planning and monitors patient progress. Coordinates with the rest of the mental health team, including doctors and nurses, to establish these plans.

CPC facilitates community-based assessment and treatment services for forensic clients across the nation. The Forensic Case Manager is instrumental in our clients' treatment and recovery. Forensic case managers works with individuals that are involved or have been involved in the criminal justice system and have a mental health diagnosis. Forensic case managers work with collaboratively with the treatment team and with the clients to link them with the information, support, and services they need to ensure that they have everything they need to prevent recidivism and receive all the supports and resources available to them. Education & Licensing Minimum Requirements:
Bachelor's degree in a behavioral health or criminal justice field (psychology, social work, counseling, criminal justice, etc.) and one year field work experience or appropriate internship; or Work under the supervision of a licensed/certified psychiatrist, psychologist, or masters or doctoral level practitioner who meets the standards of practice established by his/her state's professional regulatory board. Understanding of risk management & mitigation principles. Strong interpersonal skills and understanding of non-confrontational communication. Comfort is working directly with forensic clients and probation staff. Strong verbal and written communication skills. Proficient in usage of common computer-based operating systems. Forensic Case Manager:
Facilitate service linkage in the community and coordinate integrated services from multiple providers when applicable; Supportive problem-solving counseling; Direct service support including assistance with obtaining transportation, housing, financial support, coordinating team meetings, filing application for services (including Social Security and other local assistance programs), escort to appointments, medication compliance, and daily living skill remediation; Skill teaching to family members and other support persons; Crisis intervention as needed; Attend Re-entry/Specialty Court proceedings, as requested; May act as a liaison with insurance carriers, as requested; Evaluate clients' needs and determines if referrals to other programs or facilities are needed; Ability and willingness to travel between regions, as needed. Assist therapy clients in joining online weekly medication monitoring sessions with psychiatric providers; Engage in consistent communication with operations staff regarding scheduling; Work in an independent setting with strong organizational skills and attention to detail; Attend trainings and agency meetings, as required; Adhere to professional and confidentiality requirements in handling client records; Perform other duties as assigned.
