The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy in various settings. This position requires a strong background in behavioral health, social work, or related fields. The Case Manager will work collaboratively with clients to develop personalized care plans that address their unique needs and challenges. Duties Greet new admissions, assist in the intake process, and send introduction emails. Conduct comprehensive assessments to identify client needs and strengths. Develop, implement, and monitor individualized care plans in collaboration with clients and their families. Facilitate access to community resources, including health, nutrition, physical health care, and any additional needs outside of clinical support. Maintain accurate documentation of client interactions, progress notes, and case management activities. Work collaboratively with the clinical team and other care providers Advocate for clients' rights and needs within the healthcare system and community organizations. Maintain collateral contacts with the individual's significant others (e.g., parents, friends and siblings) to promote implementation of the service plan and optimal community adjustment. Send thorough updates to families and referents weekly. Requirements Bachelor's degree in Social Work, Psychology, or a related field; Master's degree is strongly desired. Experience in behavioral health. Strong skills in crisis intervention and mentoring. Excellent communication skills, both verbal and written. Ability to work effectively with clients and maintain confidentiality. Proficient in case management software and Microsoft Office Suite. Valid driver's license may be required for travel between client locations.
